How to Order

Fill out the attached order form and submit it to us.

We will e-mail or fax you an order confirmation and:

	Commercial invoice (with banking info for electronic transfer of funds)
	Official quote (if you requested it)
	Charge authorization form (for credit card payment)

Submit funds to 5DT via electronic transfer or credit card. For credit card payment you need to fax us your completed charge authorization form.

We accept the following credit cards:
-5DT Inc. (US and Americas): VISA, MasterCard, AMEX, Discover.
-5DT (Rest of World): VISA, MasterCard.

Once we have received the funds or credit card charge authorization, we will ship your units.
